Thank you for choosing Manassas Spine & Sport. If this is your first visit, you may want to print out and complete our New Patient Form prior to your first visit.
To set up an appointment with Manassas Spine & Sport, please note the following:
- If necessary obtain a prescription from your physician (Most chiropractic patients do not require a referral).
- If your insurance requires a referral from your Primary Care Physician (PCP), have your PCP's referral coordinator complete the referral to Manassas Spine & Sports rehab.
- If you do not have insurance, we accept most major credit cards. Payment is expected each visit unless other arrangements are made.
- After the prescription is obtained, call Manassas Spine & Sports Rehab at (703) 368-8800. Your insurance card and referral will be needed to schedule your first appointment.
- If you did not print out and complete a New Patient Form from our Web site, please arrive ten to fifteen minutes early for your first visit to complete the required paperwork.
- Please bring your insurance cards and prescription. You will be presented with a written verification of your insurance limitations, exclusions, and required payments for co-pays and deductibles including your insurance percentage responsibility. You will also be presented with HIPAA forms to read and sign.
ASSOCIATED INSURANCE PROGRAMS
Most major insurance plans cover chiropractic and physical therapy services, however each individual insurance plan has to be verified by our office in order to determine this benefit. Please bring a photo ID and your insurance card to the office on your first visit.
